Extended Care Program

C-U at Home recognizes that after the client graduates from the mid-barrier program they may still need additional support. For these reason C-U at Home offers clients an opportunity to participate in the Extended Care Program.

The Extended Care Program allows clients who are integrating into the community to retain limited case management for their first six months. During this time, a client will engage with a case manager two times a month. During these meetings, the clients will share their current goals and any barriers they are facing. The case manager provides support and assistance with the client in meeting those goals.
